A support document that provides guidance on how to complete the Match Official Development Reports (Form 1, Form 2 and Form 3) and how to use the associated matrices.
Match Official Development Review and Performance Review Survey Feedback 2019/20
In August 2018 the Rugby Football Referees’ Union and England Rugby’s Match Official Development Team introduced a nationally standardised approach to supporting the development of match officials, placing the Match Official(s) at the centre of a two-way high-quality feedback process.
During January 2019 individuals were invited to provide feedback about the new process and associated resources.
Match Official Development Review Form 1
A feedback document suitable for newly qualified match officials, those who officiate occasionally or experienced referees who would benefit from a light touch Development Review.
Match Official Development Review Form 2
A feedback document suitable for experienced referees in need of a more detailed Development Review and match officials who are part of a Development Squad/Academy.
Suitable for match officials operating at Level 6 and below, the Match Official Development Report (Form 2) should be used to provide feedback on all referees participating in exchange appointments in England.
A matrix document to assist the Match Official Developer in providing high quality, constructive developmental feedback in a consistent manner that meets the individual needs of the referee involved can also be found below. This is to assist the Match Official Developer in summarising the key strengths and development areas across the key officiating aspects.
Match Official Development Review Form 3
A Performance Review document to be used to support the development of match officials operating on Regional Groups and the National Panel. Only for use at Levels 3-5.

Discipline Report Writing Guidance
To support the accurate completion of writing discipline reports, the RFU Discipline and the RFU Match Official Development teams have produced a two page Discipline Report Writing Guidance document to assist referees in ensuring that all relevant matters are covered.
RED CARD REPORT LEVELS 5 AND BELOW
The document is to be used when reporting on any red cards that have been issued at Level Five or below. The form is to be completed and returned to CB Discipline Secretary and Referee Society Discipline Officer WITHIN 48 HOURS OF THE COMPLETION OF THE MATCH.
RED CARD REPORT AGE GRADE
The document is to be used when reporting on any red cards that have been issued at age grade fixtures.
To be submitted to the appropriate Society Discipline Secretary and all other appropriate officials.
MATCH OFFICIAL ABUSE LEVELS 5 AND BELOW
The document to be completed when reporting any match official abuse from a player, coach, club official or spectator.
To be submitted to the appropriate Society Discipline Secretary.
REPORT FORMS FOR LEVELS 1-4
The below are the links required for Levels 1-4. These competitions are appointed by the RFU Professional Game Match Official Team, National League Match Official Team and its Associates. Please submit these forms within 24 hours for midweek appointments, or prior to 9am the following Monday for weekend appointments.
Referee Society Injury Report Form
To be submitted to the appropriate Society Secretary and the RFU Sports Injuries Administrator after an injury which results in the player being admitted to a hospital (this does not include those that attend an Accident or Emergency Department and are allowed home from there) or following a death which occurs during or within six hours of a game finishing.
